Case Manager - The People Concern

Job Title: Case Manager

Report to: Program Manager

Location: Not specified

Schedule: Monday-Friday, 8:00am-4:30pm

Status: Non-Exempt (Hourly), Full-Time, Non-Management

Summary

  • Assist clients in achieving housing stability, self-sufficiency, and integration into the community.
  • Work collaboratively with clients to identify and overcome obstacles to securing housing.
  • Implement strategies to support housing retention and coordinate various services.
  • Educate and demonstrate essential life skills for a successful transition to permanent housing.
Duties and Responsibilities:

  • Manage a caseload of clients with high needs requiring case management services.
  • Build relationships focused on fostering independence and enhancing health.
  • Collaborate with mental health and health service programs.
  • Engage clients using a harm-reduction approach.
  • Support the screening, intake, and enrollment processes for clients.
  • Conduct assessments and develop treatment plans.
Qualifications:

  • Bachelor's degree preferred, or equivalent relevant experience.
  • Valid driver's license and access to reliable transportation.
  • Familiarity with case management practices preferred.
  • Comfortable working with clients facing diverse challenges.
Work Environment:

  • Combination of fieldwork and office settings.
  • Regularly required to sit, stand, bend; may need to lift or carry items up to 35 lbs.
  • Must be able to work in busy environments.
  • May involve bending, stooping, twisting, and sitting throughout the day.
