Summary
- Assist clients in achieving housing stability, self-sufficiency, and integration into the community.
- Work collaboratively with clients to identify and overcome obstacles to securing housing.
- Implement strategies to support housing retention and coordinate various services.
- Educate and demonstrate essential life skills for a successful transition to permanent housing.
- Manage a caseload of clients with high needs requiring case management services.
- Build relationships focused on fostering independence and enhancing health.
- Collaborate with mental health and health service programs.
- Engage clients using a harm-reduction approach.
- Support the screening, intake, and enrollment processes for clients.
- Conduct assessments and develop treatment plans.
- Bachelor's degree preferred, or equivalent relevant experience.
- Valid driver's license and access to reliable transportation.
- Familiarity with case management practices preferred.
- Comfortable working with clients facing diverse challenges.
- Combination of fieldwork and office settings.
- Regularly required to sit, stand, bend; may need to lift or carry items up to 35 lbs.
- Must be able to work in busy environments.
- May involve bending, stooping, twisting, and sitting throughout the day.
- Ensure a safe work environment and maintain confidentiality at all times.
- Approach problem-solving with proactivity, creativity, and flexibility.
