Job Description:

We are seeking an experienced Registered Nurse to join our team as a Dialysis Care Coordinator. This role will involve delivering care to patients with end-stage renal disease or chronic kidney disease, working closely with our interdisciplinary team to develop individualized treatment plans. Our ideal candidate will have strong communication skills, be able to work effectively in a fast-paced environment, and be passionate about making a difference in the lives of our patients.

Key Responsibilities:

  • Develop and implement individualized treatment plans for patients with end-stage renal disease or chronic kidney disease
  • Collaborate with our interdisciplinary team to ensure seamless patient care
  • Monitor patient outcomes and adjust treatment plans as needed
  • Provide education to patients and their families on dialysis procedures, medication management, and self-care techniques
  • Participate in quality improvement initiatives to enhance patient care and outcomes

Requirements:

  • Degree in Nursing (A.D.N) from accredited school of nursing required; Bachelor of Science in Nursing (B.S.N); three-year diploma from accredited diploma program may be substituted for nursing degree
  • Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN) certification preferred
  • Experience in intensive care unit (ICU), critical care unit (CCU), emergency room (ER), or medical surgical (Med Surg) setting
  • Ability to work effectively in a fast-paced environment, prioritize tasks, and manage time efficiently
  • Excellent communication and interpersonal skills
