Job Description

Overview

Moments Hospice is seeking a compassionate and skilled Licensed Social Worker (LSW) to join our Social Service team. As a Hospice Social Worker, you will play a vital role in ensuring patient comfort and providing impactful care during their final days.

Key Responsibilities

  • Provide a wide range of psychosocial services to terminally ill patients and their families, including social service goals, supportive counseling, problem-solving, and community referrals.
  • Conduct regular assessments of patient and family psychosocial needs, including pre-bereavement and anticipatory grief needs.
  • Develop and implement individualized Plans of Care to meet patient and family needs.
  • Participate in weekly interdisciplinary team meetings to discuss patient care and progress.
  • Assist with making funeral arrangements, if requested, and provide counseling during the terminal illness and at the time of death, if requested.

Requirements

  • Bachelor's degree in social work education.
  • Minimum of two years social work experience in a healthcare setting or social services agency.
  • Current Social Worker license.
  • Hospice experience is a plus.
  • Knowledge of and ability to obtain community resources, including information and referral, state/federal financial programs, and eligibility requirements.
  • Understanding of Hospice philosophy and principles of death and dying.
  • Strong organizational and communication skills.
  • Valid driver's license.
