Acute Care Nurse with a Focus on Patient Experience

Job DescriptionThis position offers a unique opportunity to work in a fast-paced Long Term Acute Care setting, providing care to patients who require extended medical attention.
As an Acute Care Nurse, you will be responsible for assessing patient needs, developing individualized care plans, and implementing evidence-based interventions to promote optimal patient outcomes.
Key responsibilities include:
- Providing direct patient care, including administering medications, performing various procedures, and collaborating with interdisciplinary teams to ensure comprehensive care.
- Assessing patient needs and developing personalized care plans to address their physical, emotional, and spiritual requirements.
- Maintaining accurate and timely documentation of patient interactions, assessments, and interventions.
To succeed in this role, you will need:
- A current RN license in good standing, with at least 18 months of recent experience in a Long Term Acute Care setting.
- Demonstrated expertise in assessing and managing complex patient conditions, such as pulmonary HTN, respiratory failure, COPD, CHF, and A-Fib.
- Strong communication and interpersonal skills, with the ability to effectively collaborate with patients, families, and healthcare teams.
- Proficiency in using electronic medical records and other relevant technology systems.

- An estimated salary of $ $130,000 - $150,000 per year, based on 36 hours of work per week and 12-hour shifts.
