CenterWell Senior Primary Care provides proactive, preventive care to seniors, including wellness visits, physical exams, chronic condition management, screenings, minor injury treatment and more.
Our unique care model focuses on personalized experiences, taking time to listen, learn and address the factors that impact patient well-being.
Our integrated care teams, which include physicians, nurses, behavioral health specialists and more, spend up to 50 percent more time with patients, providing compassionate, personalized care that brings better health outcomes.
About the RoleWe are seeking a skilled and compassionate Physician to join our team at CenterWell Senior Primary Care.
As a Physician, you will be responsible for providing high-quality, patient-centered care to our senior patients, including conducting physical exams, diagnosing and treating medical conditions, and developing personalized care plans.
You will work closely with our interdisciplinary care teams to coordinate patient care, communicate with patients and families, and participate in quality improvement initiatives.

Responsibilities- Conduct physical exams and diagnose and treat medical conditions
- Develop personalized care plans for patients
- Coordinate patient care with interdisciplinary care teams
- Communicate with patients and families
- Participate in quality improvement initiatives
- MD or DO degree
- Current medical licensure
- Board certification in Family Medicine, Internal Medicine, or Geriatric Medicine
- Experience in primary care or geriatrics
