Advanced Practice Clinician, Integrated Care Specialist

The JobThe Advanced Practice Clinician will play a critical role in providing and coordinating primary care to SCAN members. This includes conducting physical exams, assessing patient needs, and developing comprehensive plans of care. The ideal candidate will have a strong background in geriatric care and be able to work collaboratively with our case management team to ensure timely access to necessary services.
Key Responsibilities- Provide primary care to assigned members, including conducting physical exams and assessments
- Develop comprehensive plans of care for patients and their families
- Collaborate with case management team to ensure timely access to necessary services
- Manage visitation schedule in line with established protocols
- Engage with Primary Care Physicians and other healthcare professionals to promote disease prevention and health promotion
- Connect with specialists, hospitals, and community-based organizations to ensure continuity of care
- Document member visits and interactions in a timely manner
- Participate in on-call program and virtual health visits as needed
- Master's degree in Nurse Practitioner Certificate or Master's from an accredited PA program
- Current California Nurse Practitioner license or California Physician Assistant Certification (PA-C)
- Knowledge of geriatric care and chronic disease management
- Ability to identify health care needs and refer and coordinate with other resources/programs appropriately
- Knowledge of HCC and ICD-10 coding and documentation
- Knowledge of wound care and management
- Knowledge of CMS guidelines and Medicare Advantage Managed Care
- Competitive salary range: $125,400.00 to $200,600.00 annually
