Telecare Corporation is seeking a dedicated and compassionate Direct Care Professional to join our team at Stephanie House in Garden Grove, CA. As a Direct Care Professional, you will play a vital role in providing support and case management for individuals with developmental disabilities and other co-occurring disorders.

Key Responsibilities:

  • Provide direct care and support to individuals with developmental disabilities and other co-occurring disorders
  • Assist with daily living activities, such as personal care, meal preparation, and household chores
  • Develop and implement individualized care plans to promote independence and well-being
  • Collaborate with interdisciplinary teams to ensure comprehensive care and support
  • Document progress and maintain accurate records

Requirements:

  • High School Graduation or G.E.D.
  • Six months of prior experience providing direct care and special services to persons with developmental disabilities
  • Valid and current driver's license, and personal vehicle insurance with your name listed as a driver
  • Willingness to use your personal vehicle to drive clients to appointments and groups
