Behavioral Health Therapist

We are seeking a highly skilled Behavioral Health Therapist - MSW to join our team at Mid Michigan Health. As a key member of our primary care team, you will play a vital role in providing high-quality behavioral health services to our patients.
Key Responsibilities- Assessment and Treatment Planning: Conduct thorough assessments of patients referred by primary care providers, identify at-risk patients, and develop effective treatment plans to address their behavioral health needs.
- Collaboration with Primary Care Team: Work closely with primary care providers to recognize and treat mental disorders and psychological problems, and refer cases to other behavioral health providers as needed.
- Patient Engagement and Follow-up: Facilitate patient engagement and follow-up care, and work with the primary care team to treat and manage patients with chronic emotional and physical health problems.
- Brief Behavioral Interventions: Provide brief behavioral interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments as appropriate.
- Documentation and Data Management: Systematically document treatment response and monitor patients for changes in clinical symptoms and treatment side effects or complications, and use data to guide treatment and follow-up.
- Program Development: Assist in the development of programs to address specific behavioral health needs, such as pain management, diabetes education, and stress reduction.
- Patient Education and Resources: Provide and update patient education materials about common mental health and substance use disorders, and connect patients with local treatment options and resources.
- Registry Management: Track patient follow-up and clinical outcomes using a registry, and document patient progress and treatment recommendations in the Electronic Health Record (EHR).
- Education: Master's Degree in Social Work (MSW) from an accredited college or university.
- Licensure: Licensure as a Licensed Psychologist or Master's Social Worker in the State of Michigan.
- Experience: Membership in at least one professional organization, such as the American Psychological Association (APA) or the National Association of Social Workers (NASW), is preferred.
- Skills: Strong interpersonal skills, ability to communicate effectively with individuals and groups, and knowledge of Integrated Behavioral Health practices, processes, and procedures.
