Key Responsibilities:
- Conduct in-home assessments, including medical history review, vital signs, and comprehensive physical exams
- Perform evidence-based practice screenings and point-of-care testing as needed
- Identify diagnoses and develop care management plans to improve patient outcomes
- Communicate findings with patients' primary care physicians to ensure seamless care coordination
- Intervene in urgent and emergent situations, providing timely and effective care
Requirements:
- Active NP license or ability to obtain by start date
- ANCC or AANP national certification in Family, Geriatrics, Adult, or Adult-Gerontology Primary Care
- Ability to complete physical requirements of the job, including lifting and navigating various dwelling conditions
- Reliable transportation for home visit assessments
Preferred Qualifications:
- 1+ years of clinical experience in a related setting
- Experience in gerontology, cardiology, internal medicine, or endocrinology
- Home health care or home visit experience
- Fluency in Spanish, Cantonese, Korean, Vietnamese, Polish, or other languages
