Overview

Liberty Health is seeking a skilled Care Coordinator to join our team. As a key member of our interdisciplinary team, you will play a vital role in ensuring the highest quality care for our patients and their families.

About the Role

This is a full-time position that requires strong interpersonal skills, effective communication, and the ability to work autonomously. You will be responsible for conducting comprehensive assessments, developing patient-centered care plans, and providing education to patients and their families.

As a Care Coordinator, you will engage with patients and their loved ones to identify their unique needs and develop tailored care plans that address their social, emotional, financial, and environmental factors. You will also participate in the orientation and training of new employees and volunteers, promoting teamwork and shared responsibility for achieving our agency mission and vision.

JOB RESPONSIBILITIES
  • Patient Assessments: Conduct thorough psycho-social assessments to identify patient and family needs, including bereavement risk, coping skills, and social, spiritual, and cultural factors.
  • Care Planning: Develop, maintain, and assist patients and families in establishing patient-centered care plans, including treatment goals and interventions.
  • Education and Support: Provide education and support to patients and their families on accessing community resources, managing grief, and navigating the healthcare system.
  • Bereavement Program Management: Oversee the implementation of bereavement programs and services to meet the needs of patients, families, and the community.
QUALIFICATIONS
  • Degree: Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education, or Baccalaureate of Social Work (BSW) degree from an institution accredited by the Council on Social Work Education, or a baccalaureate degree in psychology, sociology, or other field related to social work.
  • Experience: One year of experience in a health care setting, preferably in hospice care.
  • Skills: Strong interpersonal skills, effective communication, organization, timeliness, and the ability to provide effective care for patients and families.
