Medical Care Specialist

We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our team at Nonotuck Resource Associates Inc. This rewarding role will involve providing comprehensive case management services to individuals with developmental disabilities, ensuring their unique needs are met with dignity and respect.
About the Role:
As an LPN, you will be responsible for conducting thorough intake assessments, developing and implementing individualized care plans, and providing supervision and training to caregivers. Your excellent communication skills and ability to work collaboratively with families, caregivers, and other stakeholders will be essential in delivering high-quality care.
Key Responsibilities:
• Conduct thorough intake assessments and develop individualized care plans tailored to each member's unique needs
• Provide supervision, training, and evaluation of caregivers to ensure high-quality care delivery
• Collaborate with caregivers, families, and other stakeholders to ensure seamless care coordination
• Attend various meetings and contribute to the development of effective care strategies
• Participate in on-call rotations and provide support as needed during non-business hours
Requirements:
To be successful in this role, you will need a valid MA LPN license, computer literacy, and experience working with individuals with developmental disabilities. A strong understanding of community-based services and HCBS waivers is beneficial. You must also possess excellent interpersonal and communication skills, with the ability to work effectively in a team environment.
