Job Summary

We are seeking a skilled Clinical Social Worker to join our team at Mass General Brigham. As a key member of our ambulatory primary care team, you will work closely with clinical and administrative staff to enhance the delivery of patient care services.

Key Responsibilities
  • Conduct comprehensive assessments and case management to identify high-risk psychosocial factors of patients.
  • Collaborate with primary care staff to understand the influence of psychosocial factors on medical care and assume care coordination responsibility of patients.
  • Provide treatment or intervention as needed, or work with treating outpatient clinicians to ensure effective treatment.
  • Develop and implement health promotion strategies, crisis management plans, and self-management education programs.
  • Provide transitional care, patient and family support, liaison to community services, short-term counseling, and advanced care planning.
  • Participate in team consultation and contribute to the development of care plans.
Requirements
  • Current LICSW/LCSW in MA.
  • Minimum of Masters-level degree program in social work.
  • Minimum of 2 years clinical experience, with post-acute care settings or care coordination/case management experience a plus.
