About the Role

We are seeking experienced Home Health RNs to join our team. As a Home Health RN, you will provide one-on-one care to patients in their own homes, working with a team of physicians and overseeing LPNs, CNAs, and aides. Your responsibilities will include:

  • Examining patients, doing assessments, and documenting progress
  • Reporting changes to physicians and educating patients on their plan of care
  • Coaching family members on patient care and home safety
  • Ensuring goals are met and coordinating discharge from services
Requirements

To be successful in this role, you will need:

  • Active Registered Nurse (RN) License in Colorado
  • Graduate of an accredited school or program of nursing
  • At least 1200 hours of experience as an RN
  • Healthcare CPR/BLS Certified
  • Home health care experience preferred
