**Key Responsibilities:**

* Administer personal care and related services to patients in their homes
* Collaborate with healthcare teams to develop and implement personalized care plans
* Provide emotional support and companionship to patients and their families
* Maintain accurate records and communicate effectively with patients, families, and healthcare teams

**Requirements:**

* High school diploma or equivalent required
* Six months of experience working with people and a passion for working with others
* Nursing Assistant or Home Health Aide certification required
* American Heart Association Basic Life Support (BLS) certification required
* Preferred experience in Home Health setting and certifications in Home Health Aide or Long Term Care Aide
