Physician Assistant for Home-Based Rehabilitation Services

Job Summary: As a Physician Assistant for Home-Based Rehabilitation Services, you will play a vital role in delivering high-quality patient care in the comfort of patients' homes. You will work closely with our multidisciplinary team to develop and implement individualized treatment plans, utilizing your expertise in musculoskeletal and neurological rehabilitation.
Responsibilities:
- Conduct thorough assessments of patients' physical abilities and create customized treatment plans to address their unique needs.
- Develop and maintain effective working relationships with patients, families, caregivers, and healthcare professionals to ensure seamless coordination of care.
- Provide education and training to patients and their support systems on proper use of equipment, exercises, and self-care techniques.
- Maintain accurate and detailed records of patient progress, including evaluation results, treatment plans, and outcomes.
- Participate in regular team meetings to discuss patient cases, share best practices, and receive feedback from colleagues.
Requirements:
- Graduate of an accredited physician assistant program with certification from a recognized professional organization.
- Current licensure as a physician assistant in good standing with the state licensing authority.
- Valid driver's license with reliable transportation to visit patients at home.
- Minimum 2 years of experience in acute care or home health settings, with a focus on musculoskeletal and neurological rehabilitation.
- Strong communication and interpersonal skills, with ability to work effectively with diverse populations.
- Ability to lift up to 50 pounds frequently and occasionally lift over 50 pounds, with provision of reasonable accommodations as needed.
