Your Role in Patient Care:

The RN Care Manager collaborates with patients, families, social workers, nurses, physicians, and the interdisciplinary team to ensure patient-centered care coordination and seamless progression through the healthcare continuum. This role is essential in promoting efficient and cost-effective care through resource management and clinical care interventions.

Your Contributions to the Team:

  • Conducts Initial Evaluations for transition of care needs for all identified patients within one calendar day of admission, documenting in accordance with established policies and procedures. Engages with patients and their caregivers (as permitted) while reviewing current and historical inpatient and outpatient medical records during the Initial Evaluation.
  • Assesses necessary patient information, including laboratory results, medications (pre and post-hospital), History and Physical, therapy notes, emergency department notes, test results, and progress notes.
  • Integrates patient and family care goals and preferences into the transition of care planning, effectively communicating these to the multidisciplinary team.
  • Incorporates clinical, social, and financial considerations into the transition of care plan.
  • Meets with patients and families to discuss realistic and suitable discharge options and providers for post-hospital care.

Qualifications for Success:

  • Minimum Requirements:
  • Associate Degree in Nursing
  • Current Registered Nurse (RN) License
  • Two (2) years of experience in medical/hospital nursing
  • Preferred Qualifications:
  • ACM/CCM Certification
  • Master's degree in a health-related field or MSN
  • Previous experience in Care Management or Utilization Management
