About the Role

We are seeking a skilled Speech Language Pathologist to join our team at CenterWell Home Health. As a Speech Language Pathologist, you will play a vital role in providing high-quality care to our patients in the comfort of their own homes.

Key Responsibilities
  • Evaluate and provide speech/language pathology services to patients in their homes or facilities
  • Develop and implement individualized treatment plans to address patients' communication and swallowing disorders
  • Collaborate with healthcare teams to ensure comprehensive care and coordination
  • Provide education and training to patients and their families on effective communication strategies and techniques
  • Document patient progress and outcomes in a timely and accurate manner
Requirements
  • Master's degree in Speech-Language Pathology or related field
  • Current licensure and certification in Speech-Language Pathology
  • Minimum of six months experience as a Speech-Language Pathologist
  • Home health experience preferred
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
