RN Case Manager Hospice Position

We are seeking a skilled RN Case Manager to join our team at Blossom Ridge Hospice in Sacramento, CA. As a Hospice RN, you will have the opportunity to develop trusting relationships with patients and their families, providing end-of-life care with compassion and empathy.
Responsibilities
As the RN Case Manager, you will be responsible for delivering high-quality care to our patients, including:
- Conducting initial nursing evaluations to determine eligibility for hospice services
- Identifying patients' physical, psychosocial, and environmental needs and reassessing as needed
- Initiating and coordinating the plan of care, maintaining accurate clinical notes, and documenting patient progress
- Collaborating with patients, families, attending physicians, and other members of the IDG to provide comprehensive care
Qualifications
To be successful in this role, you will need:
- One year of clinical experience as an RN
- Current RN licensure in the state of practice and one year of clinical experience
- Current CPR Certification
- Current driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation
