Geriatric Care Specialist

We are seeking a skilled Geriatric Care Specialist to join our team in California. This is a full-time position that offers comprehensive evaluations of elderly patients, including their physical, cognitive, psychosocial, and functional abilities.
The successful candidate will perform assessments and provide ongoing medical care to participants. They will also work with doctors and the interdisciplinary team to create individualized care plans for each participant.
The ideal candidate will have a Master's degree in nursing, at least two years of senior population experience, and an active registered nurse license within the jurisdictional region. They should also have up-to-date knowledge of chronic care and geriatric issues, as well as best practices.

About the Role- Conduct in-person assessments and provide ongoing medical care to participants.
- Create individualized care plans for each participant in collaboration with the interdisciplinary team.
- Assist in providing care to nursing home residents by making regular visits based on their needs.
- Collaborate with the doctors, community-based PACE doctors, and the interdisciplinary team to oversee and ensure effective care coordination.
