Licensed Vocational Nurse I

Job Summary:
As a Licensed Vocational Nurse I, you will play a vital role in providing specialized care to individuals residing at our SH. You will work under the guidance of a Registered Nurse, performing routine vocational nursing work, including administering medications, treatments, and providing basic and routine nursing tasks. Your expertise will be essential in promoting the health and well-being of our patients, while fostering a collaborative and supportive team environment.
Responsibilities:
1. Administer medications and treatments orally, rectally, internally, or by injection, as prescribed by a physician or Advanced Practice Registered Nurse (APRN).
2. Assist other medical staff, such as Registered Nurses (RNs), Physicians, Advanced Practice Registered Nurses, and Physician Assistants, in examining and caring for patients.
3. Make rounds and/or home visits as needed, working in other locations at the SH as required.
4. Demonstrate competency in performing technical nursing tasks and procedures in accordance with HHSC, SH, and state regulations.
5. Assist in acclimating and orienting new LVN staff to perform nursing functions at the SH, including introducing them to the individuals and describing their needs and preferences.
6. Preform basic and routine nursing tasks according to facility and state regulations.
7. Monitor individuals' weight, diet, mealtimes, and vital signs.
8. Timely and accurately document and report pertinent treatments and observations in the individual's medical record (electronic health record or paper chart) in accordance with SH procedures and protocols.
9. Fosters interdisciplinary collaboration and integration. Communicates pertinent information orally and in writing to other team members, such as the RN Case Manager, in order to ensure the treatment team has the best information possible to make decisions.
10. Responds to emergency situations to ensure safety of individuals. Follows safety and infection control procedures of the SH.
11. Collects specimens for laboratory examination and conducts appropriate screening tests when requested by lab personnel.
12. Provides information and education to individuals, in accordance with their care plan, when performing procedures and treatments such as explaining the name and purpose of a particular medication and explaining why the individuals is taking the medication.
13. Ensures the patient is positioned correctly prior to providing a treatment and, when necessary, assists and/or instructs other staff in turning/repositioning individuals to administer treatments.
14. Ensures that supplies are available, in advance, to provide the required procedure or treatment. Takes inventory and ensures the home/unit is properly stocked. Orders supplies through appropriate channels when they are needed.
Requirements:
1. Current licensure as a Licensed Vocational Nurse in the State of Texas.
2. Ability to prepare and maintain records and to use an electronic health record system once trained.
3. Ability to communicate effectively and to instruct others.
4. Must maintain current CPR certification.
5. Must have strength and flexibility to safely perform Cardiopulmonary Resuscitation (CPR).
