Our facility boasts 204 inpatient beds and offers a wide range of services, including cancer care, cardiovascular services, neuroscience, advanced gastrointestinal care, and wound healing.

Position Summary
The Case Manager will oversee a designated caseload, ensuring timely assessment, planning, implementation, and evaluation of discharge plans and care transitions throughout the continuum of care. This role is essential for optimizing the use of both patient and Health System resources while adhering to CMS and other external review agency requirements.
- Coordinate case management needs, including care transitions and discharge planning, while providing consultations, advocacy, and education.
- Maintain effective communication with healthcare team members regarding assessment findings, discharge planning requirements, and necessary provider orders for arranging home care, durable medical equipment, transportation, skilled nursing, or rehabilitation facility placement, substance abuse treatment, and outpatient follow-up.
- Stay informed about specific benefits and reimbursement guidelines, the discharge planning process, and relevant federal, state, and local regulations.
- Participate in Quality Assurance/Performance Improvement (QAPI) initiatives as needed.
- Responsibilities may include site-specific duties, weekend/holiday, or after-hours coverage as designated by the entity.

- Excellent written and verbal communication skills.
- Basic computer proficiency.
Minimum Qualifications
Education: A Bachelor of Science in Nursing (BSN) or Master of Social Work (MSW) is required.
Experience: A minimum of three years of relevant experience is required.
Licensure and Certification: Case Management Certification (ACM, CCM, or ANCC) is required within three years of hire.
