About the Role:

We are seeking a skilled Registered Nurse to join our team at Grace Healthcare. As a Case Management/Assessment and Supervisory Visits RN, you will be responsible for meeting with clients in their homes or facilities for assessments and periodic monitoring.

  • Ability to travel to cases as assigned within the Wake County area
