Job Summary

The Duke Health team is seeking an experienced Advanced Practice Provider to join our Internal Medicine department. As a key member of our interdisciplinary team, you will be responsible for providing high-quality patient care in a primary care setting.

Key Responsibilities

  • Coordinate Patient Care: Work collaboratively with the primary health care team to ensure optimal patient care services, providing initial and ongoing assessment and treatment of patients' medical, physical, and psychosocial status.
  • Coordinate Diagnostic Procedures: Schedule laboratory studies, diagnostic procedures, and services such as labs, x-rays, and referrals, ensuring seamless coordination and communication with patients and their families.
  • Effective Communication: Explain the necessity, preparation, nature, and anticipated effects of scheduled diagnostic and therapeutic services to patients and their families, promoting informed decision-making and patient engagement.
  • Documentation and Record-Keeping: Accurately document in patients' medical records all relevant data, including results of diagnostic tests, laboratory results, patients' condition, and response to therapies/interventions.
  • Teamwork and Collaboration: Demonstrate excellent communication skills and work effectively in a team environment, fostering a culture of collaboration and respect.
  • Work Schedule: Commit to a Monday - Friday schedule, providing up to 36 hours of direct patient care, and maintaining a high level of productivity and efficiency.

Requirements

  • Licensure and Certification: Possess the necessary licensure and certification to care for adult patients.
  • Excellent Communication Skills: Demonstrate exceptional communication skills, both written and verbal, to effectively interact with patients, families, and healthcare teams.
