About the Role

This is a great opportunity to work as an RN Care Coordinator in our team at Optum. We are looking for a skilled professional who can provide high-quality care to our members.

As an RN Care Coordinator, you will be responsible for coordinating care services for our members in the Multnomah and Washington counties. You will work closely with our primary care team, including physicians, nurses, and other healthcare professionals, to ensure that our members receive comprehensive and coordinated care.

Key Responsibilities:

  • Establishing and maintaining relationships with members, family members, caregivers, and other healthcare professionals;
  • Coordinating care services, including scheduling appointments, arranging transportation, and communicating with providers;
  • Conducting regular visits to assess member needs and develop personalized care plans;
  • Collaborating with the healthcare team to ensure seamless care transitions and efficient use of resources;
  • Documenting member information and tracking progress towards goals;
  • Maintaining confidentiality and adhering to HIPAA regulations;
  • Staying up-to-date with Model of Care (MOC) documentation and policy updates;
  • Familiarizing yourself with clinical resources, including Resource1, iCoreCodeGenius, Sparq, Learnsource, and other Optum internal sites;

Requirements

To be successful in this role, you will need:

  • A current unrestricted Registered Nurse license in Oregon;
  • 3+ years of RN experience, preferably in case management or post-acute care;
  • 1+ year of experience working with electronic medical records;
  • Basic Microsoft Office skills and ability to navigate in a Windows environment;
  • Excellent communication and customer service skills;
  • A valid driver's license and ability to travel locally 75% of the time;

Salary and Benefits

We offer a competitive salary of $87,500 - $105,000 per year, based on experience, plus benefits, including health insurance, retirement plan, and paid time off.
