The Licensed Practical Nurse (LPN) is responsible for providing and documenting skilled nursing care, under the supervision of a Registered Nurse, in accordance with the developed care plan and physicians orders for each individual patient while adhering to confidentiality standards and professional boundaries at all times.
Essential Job Functions:- Deliver and Coordinate Quality Patient Care: Provide high-quality patient care, following physician orders and care plans.
- Continuously Observe and Assess Patient Condition: Monitor patient condition and report changes to the supervisor and/or physician as necessary.
- Document All Activities and Assessments: Accurately document all patient interactions, assessments, and care provided.
- Participate in Nursing Care Planning: Collaborate with the healthcare team to develop and update patient care plans.
- Take Appropriate Nursing Action: Respond to patient needs and take necessary actions to achieve expected outcomes.
- Recognize and Respond to Changes in Patient Needs: Identify changes in patient condition and implement care to prevent risk or reduce risk.
- Accept Responsibility for Personal and Professional Accountability: Comply with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act.
- Provide Care Utilizing Infection Control Measures: Follow OSHA standards to prevent the spread of infection.
- Educate Patients and Families: Teach patients and families about disease management, self-care techniques, and prevention strategies.
- Maintain Competencies: Stay up-to-date with the latest skills and knowledge in the field through ongoing education and training.
- Graduate of an Accredited School of Nursing: Hold a degree from an accredited nursing program.
- Current, Unrestricted State License: Possess a current, unrestricted state license as a Licensed Nurse in the state of practice.
- CPR Certification: Hold current CPR certification.
- Demonstrated Proficiency in Clinical Assessments: Show proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
- Recent Experience as a Licensed Nurse: Have six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
- Home Health Experience: Possess experience working in home health care.
- Attention to Detail: Possess strong attention to detail.
- Time Management: Demonstrate effective time management skills.
- Effective Problem-Solving and Conflict Resolution: Show ability to resolve conflicts and problems effectively.
- Good Organization and Communication Skills: Possess strong organizational and communication skills.
- English Language Proficiency: Possess the ability to speak, write, read, and understand English.
- Travel Ability: Be able to travel as needed.
- Lifting Ability: Be able to lift 50 pounds.
- Physical Demands: Meet the physical demands of the job, including prolonged walking, standing, bending, kneeling, reaching, twisting, sitting, and climbing stairs.
- Varied Environments: Be able to function in a wide variety of environments, including exposure to allergens and other conditions.
- Possible Exposure to Blood and Infectious Diseases: Possess the ability to work in environments where there may be exposure to blood and infectious diseases.
