- Deliver patient care and education tailored to each patient's age, physical ability, educational level, and culture
- Conduct health assessments and evaluate patient needs in the primary care clinic
- Perform physical exams, order and/or perform laboratory and diagnostic tests, and make clinical judgments regarding results
- Independently perform minor surgical procedures
- Develop comprehensive care plans for each patient
- Initiate appropriate consultations and referrals
- Provide health information and teaching with an emphasis on prevention and curative treatment
- Act as a primary care resource to the Pediatric Multidisciplinary Team
- Participate in educational activities with nurses, nurse practitioners, and medical students
- Document history, physical assessments, and treatment interventions in medical records
- Engage in committees for quality of care, standards of care, policies and procedures, and program development activities
- Maintain current standards of practice and pursue professional development
- Attend regularly scheduled clinical care, team, and department meetings
- Perform other duties assigned by the physician and/or supervisor as required
- Current certification and licensure as a Nurse Practitioner in the Commonwealth of Massachusetts
- Ability to maintain patient confidentiality at all times as required by the health center and regulations
- Experience working with neonatal, pediatric, adolescent, and young adults up to age 21
- Experience in culturally diverse settings and a passion for community health
- Proficient in Spanish, Portuguese, and/or Cape Verdean is desirable
