Nurse Case Manager

We are seeking a skilled and compassionate Registered Nurse to join our team as a Case Manager in Laconia, NH. As a key member of our healthcare team, you will be responsible for managing a defined caseload of patients, providing direct patient care, and delivering comprehensive nursing services to individuals in the community.
Responsibilities:
- Manage a defined caseload of patients, providing direct patient care and comprehensive nursing services.
- Collaborate with healthcare professionals to develop and implement individualized care plans.
- Conduct regular assessments and evaluations to ensure optimal patient outcomes.
- Provide education and support to patients and their families to promote health and wellness.
Requirements:
- Valid nursing license in the state of NH.
- ~2+ years of experience in skilled care preferred.
- ~1+ year of experience in nursing supervision preferred.
- ~Recent wound care experience.
