Job Summary

Tenet Massachusetts is seeking a skilled Registered Nurse to fill a Case Management position at one of our facilities.

The ideal candidate will have a strong background in acute care and experience with case management principles. This role will involve coordinating care along a continuum to ensure patients receive the right level of care at the right time.

The successful candidate will be responsible for assessing patients for transition needs, promoting timely throughput, and preventing avoidable readmissions. They will also be responsible for ensuring compliance with state and federal regulatory requirements, as well as Tenet policy.

Requirements:

  • Graduate of an accredited School of Nursing
  • Current RN licensure in State of Massachusetts
  • 2 years RN acute care hospital experience
  • Acute care case management experience preferred
