Job Summary

We are seeking a highly skilled Pediatric Physical Therapist to join our team at Connect Home Health. As a Pediatric Physical Therapist, you will play a vital role in providing comprehensive and well-coordinated care to pediatric patients in the comfort of their own homes.

As a member of our team, you will have the opportunity to build lasting relationships with patients and families, and contribute to our mission of providing support and stability to families of medically fragile children.

Key Responsibilities
  • Perform comprehensive evaluations to determine pediatric patients' needs for therapy.
  • Develop and implement relevant treatment plans aligned with physician's orders.
  • Provide in-home therapy services for pediatric patients with a range of diagnoses.
  • Engage with patients' families, providing regular communication and updates on goals for their child.
  • Collaborate with a multidisciplinary team to develop holistic treatment plans that address the diverse needs of each patient.
  • Maintain comprehensive documentation that reflects the care provided and communicates changes in treatment plans to the entire healthcare team.
Requirements
  • A valid Physical Therapy license in the state of Texas.
  • Current BLS License.
  • Reliable transportation, a valid driver's license, and automotive insurance.
  • Pediatric experience is preferred but not required.
