Job Description:

Connect Home Health LLC is seeking a skilled Pediatric Occupational Therapist to provide comprehensive in-home therapy services for medically fragile and special needs children.

As a member of our team, you will play a vital role in ensuring that pediatric patients receive well-coordinated care in the comfort of their own homes.

Clinicians at Connect Home Health LLC build lasting relationships with their patients and families, providing personalized care that addresses the unique needs of each child.

Responsibilities:
Perform comprehensive evaluations to determine pediatric patients' needs for therapy.
Develop and implement relevant treatment plans aligned with physician's orders.
Provide in-home therapy services for pediatric patients with a range of diagnoses.
Engage with patients' families, providing regular communication and updates on goals for their child.
Collaborate with a multidisciplinary team to develop holistic treatment plans that address the diverse needs of each patient.
Maintain comprehensive documentation that reflects the care provided and communicates changes in treatment plans to the entire healthcare team.

Requirements:
A valid Occupational Therapy license in the state of Texas
Current BLS License
Reliable transportation, a valid driver's license, and automotive insurance
Pediatric experience is preferred but not required
