Job Summary

We are seeking a compassionate and motivated Physical Therapist to join our team. As a Physical Therapist for At-Home Patients, you will work with patients in their homes to provide one-on-one physical therapy services. You will be responsible for assessing patients, developing treatment plans, and implementing interventions to improve their physical function and mobility.

Responsibilities
  • Assess patients in their homes to identify their physical therapy needs and develop personalized treatment plans
  • Implement interventions to improve patients' physical function and mobility, including therapeutic exercises, joint mobilization, and modalities
  • Document patient progress and communicate with patients, families, and healthcare providers as needed
  • Collaborate with interdisciplinary teams to ensure comprehensive care and coordinate services
  • Provide education and training to patients and their families on proper use of equipment and techniques
Requirements
  • Master's or Doctoral degree in Physical Therapy
  • Current licensure as a Physical Therapist in the state of practice
  • Experience working with patients in home-based settings
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
