Emerus is seeking a skilled Nursing Care Coordinator to join our team. In this role, you will play a vital part in coordinating patient care and ensuring seamless transitions between departments.

**Job Description:

As a Nursing Care Coordinator, you will be responsible for:

  • Assessing patient needs and developing individualized care plans
  • Coordinating with interdisciplinary teams to ensure timely and effective care
  • Maintaining accurate records and communicating with patients and families
  • Identifying and mitigating risks to patient safety and well-being

**Responsibilities:*

  • Collaborate with physicians and other healthcare professionals to coordinate patient care
  • Develop and implement effective care plans that meet patient needs
  • Ensure timely and efficient communication between departments
  • Identify and address potential issues that may impact patient care

**Requirements:

To be successful in this role, you will need:

  • Associate's Degree or Nursing Diploma required; BSN preferred
  • Current state licensure as a Registered Nurse without sanctions
  • 2+ years full-time RN experience in Telemetry/ICU or related healthcare positions
  • BLS and ACLS certifications from the American Heart Association or American Red Cross
