Home Health Nurse Specialist

We are seeking a skilled Home Health Nurse Specialist to join our team. As a Registered Nurse, you will provide exceptional medical care to geriatric clients living in Assisted Living and Independent Living communities in Wakefield, Woburn, Danvers, and surrounding towns.
Responsibilities- Follow a designated care plan in accordance with patient's medical needs
- Make visits to clients in designated geographic territories
- Perform assigned duties, including wound care, treatments, and procedures
- Monitor clients' conditions; reporting changes to Clinical Manager
- Follow up with, execute, and properly document doctors' orders
