Position: Practical Nursing Specialist
In this rewarding role, you will advance your nursing career while making a significant impact on the lives of those we serve. Your responsibilities will include delivering essential nursing services, training staff, and addressing the medical and psychiatric needs of individuals with various challenges.
- Monitor and assess the health conditions of individuals to guarantee high-quality care.
- Administer prescribed medications, maintain accurate documentation, and supervise staff in medication administration.
- Implement medical treatments as per physician directives and provide personal care services.
- Conduct thorough assessments and develop health care services for each Individual Service Plan (ISP), responding proactively to any changes in condition.
- Train staff on critical topics such as universal precautions, infection control, and medication administration.
- Act as a liaison and advocate, collaborating with medical professionals and external agencies to coordinate comprehensive care.
- Educate individuals on self-medication, prescribed treatments, and health-related topics such as smoking cessation and STD prevention.
- Associate's Degree in Nursing or a related field, or state certification.
- A minimum of one year of nursing experience with the population served.
- Current LPN or LVN licensure in the state of residence.
- Valid driver's license, car registration, and auto insurance.
- Additional certifications as mandated by state regulations, including CPR/FA and HIV/AIDS certification.
- Strong communication skills to effectively interact with staff, individuals served, and families.
- Proficiency in English, with the ability to comprehend complex instructions and document care effectively.
