Key Responsibilities- Develop and implement individualized care plans to ensure optimal patient outcomes
- Collaborate with healthcare providers, social services, and other stakeholders to coordinate patient care
- Conduct thorough assessments of patient needs and develop strategies to address them
- Monitor patient progress and make adjustments to care plans as needed
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
- Current Texas RN License
- One year of full-time, recent RN acute care hospital experience
- Previous case management experience preferred
- Certified Case Manager (CCM) certification preferred
- Bachelor's degree in nursing
- Pediatrics experience
- Bilingual English/Spanish
