Job Summary

We are seeking a skilled Nurse Practitioner to join our team, providing primary and consultative professional care in a variety of care settings. As a Nurse Practitioner with Careline Health Group, you will have the opportunity to work with geriatric and seriously ill populations, assisting patients and families with comfort and quality of life.

Key Responsibilities
  • Conduct medical evaluations of predominantly elderly, frail, seriously ill patients in their home environment
  • Determine follow-up assessments according to patient need and best practice guidelines
  • Communicate significant findings, problems, and changes in condition or environment to the clinical coordinator, the physician, family, facility, and other personnel involved with patient care
  • Assess health status by obtaining relevant health and medical history
  • Perform a physical exam based upon the age and patient history as per the Standard Care Arrangement with a collaborating physician
  • Order, interpret, and evaluate laboratory tests and other diagnostic studies to assess patient's clinical problems and healthcare needs
  • Perform diagnostic and therapeutic interventions consistent with certification, law, and state medical privileges
  • Prescribe and monitor medications as allowed by prescriptive authority through the appropriate State Board Agency
  • Develop a treatment plan in collaboration with the patient and caregivers
  • Refer patients to physicians as appropriate for consultation or for specialized health resources and treatment
  • Ensure continuity of care by serving as a liaison between patient and other members of the multidisciplinary care team or with other specialty areas
  • Provide patients and families with an understanding of the diagnosis and disease progression while decreasing unnecessary emergency room and hospital visits
  • Hold case conferences with families when appropriate to advance the care plan
  • Meet minimum productivity requirements based on practice standard
Requirements
  • Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
  • Master's or doctoral degree as a Board-Certified Nurse Practitioner
  • Current State License as a Registered Nurse and Nurse Practitioner
  • Nursing skills as defined by accepted nursing standards
  • Sit, stand, bend, lift, and move intermittently
  • Must be able to lift at least 35lbs and bear the weight of an average adult effectively
  • Current and valid driver's license
  • Reliable transportation and valid automobile insurance coverage
  • Demonstrates ability to assess and respond to the needs of patients, families, and caregivers in skilled-nursing facility settings
  • Demonstrates skill in nursing practice, ability to cope with family emotional stress, and tolerance of individual lifestyles
  • Ability to handle multiple priorities, documentation requirements, and deadlines
Preferred Qualifications
  • 2 years clinical experience in a post-acute or as a Nurse Practitioner preferred
  • Experience with geriatric patients preferred
  • Familiarity with Aprima EMR
Location and Hours
  • Office Location: 801 Rosehill Road, Jackson MI 49201
  • Main Service Area: Ingham/Eaton Counties and surrounding areas
  • Office hours: 8:00 AM - 5:00 PM, Monday through Friday
