Key Responsibilities- Conduct thorough medical evaluations of elderly and frail patients in their homes
- Determine follow-up assessments based on patient needs and best practice guidelines
- Communicate significant findings and changes in condition to the clinical coordinator, physician, family, and other personnel involved in patient care
- Assess health status by obtaining relevant health and medical history
- Perform physical exams and order laboratory tests as needed
- Develop treatment plans in collaboration with patients and caregivers
- Refer patients to physicians as necessary for consultation or specialized care
- Ensure continuity of care by serving as a liaison between patients and other members of the care team
- Graduate of an accredited nursing program
- Master's or doctoral degree as a Board-Certified Nurse Practitioner
- Current State License as a Registered Nurse and Nurse Practitioner
- Nursing skills as defined by accepted nursing standards
- Ability to assess and respond to patient needs in skilled-nursing facility settings
- 2 years of clinical experience in a post-acute or as a Nurse Practitioner
- Experience working with geriatric patients
- Familiarity with Aprima EMR
