Job Description:
We are seeking a dedicated and compassionate Hospice Nurse Practitioner to join our team in Pennsylvania. As a member of our interdisciplinary care team, you will play a crucial role in ensuring that patients receive high-quality medical care in the comfort of their own homes.

Responsibilities:
• Recertify and discharge patients from hospice
• Visit patients at home to assess their eligibility for hospice care
• Make treatment recommendations and write orders as needed
• Work closely with the interdisciplinary care team to ensure that patients' medical needs are met
• Document patients' recertification eligibility in the hospice program
• Represent the hospice philosophy of care to the medical community

Requirements:
• Board-licensed nurse practitioner degree, licensed to practice in NJ and PA
• Experience in family practice, oncology, or hospital practice
• Excellent communication skills, both written and verbal
• Ability to work independently and collaboratively as part of a team

Salary:
$120,000 per year
