Job Overview

We are seeking an experienced Primary Care Nurse Practitioner to join our team in Milford, PA. The ideal candidate will have a strong background in providing high-quality patient care and excellent communication skills.

About the Role:

  • Provide direct patient care, discussing symptoms and concerns, and perform thorough examinations to diagnose medical problems and prescribe medication or create treatment plans if necessary.
  • Administer treatment and observe patients' functioning over time.
  • Record medical histories of patients, perform physical exams, administer diagnostic tests, and create patient care plans.
  • Collaborate with physicians to treat advanced illnesses or injuries when required.

Responsibilities:

  1. Perform physical examinations and screenings of patients.
  2. Record patients' medical histories and symptoms.
  3. Create patient care plans and contribute to existing ones.
  4. Order, administer, and analyze diagnostic tests, including bloodwork.
  5. Review and interpret test results to diagnose health problems.
  6. Recommend further treatment options or future appointments if needed.
  7. Monitor and operate medical equipment.
  8. Administer medicine and other treatments.
  9. Provide referrals to other practitioners when necessary.
  10. Detect changes in patients' health and modify treatment plans as needed.
  11. Provide after-care and ongoing care information and resources to patients and their families.
  12. Consult with healthcare professionals and families.
  13. Train patients and their families to manage and prevent illness and injury.
  14. Ensure patient records are up-to-date, including test results, immunizations, condition changes, etc.

Requirements:

  • Minimum 1 year of recent experience as an RN in a clinical setting.
  • Active RN license within the state of practice.
  • Current certifications (variable depending on the facility).
  • Prescriptive authorization.
  • Experience providing primary care as a family nurse practitioner.
  • Knowledge of computer-based data management programs and information systems, as well as medical records and point-of-interview technology.
  • Able to operate as part of a team, alongside physicians and other nurses.
  • Proficient in EHR medical charting system and computer-based data management programs and information systems.
  • Sound understanding of all pertinent federal and state regulations, including HIPAA and OSHA.
  • Excellent verbal and written communication skills.
  • Knowledge of disease prevention.
