Healthcare Navigator

Job Summary:
The Community Health Navigator plays a pivotal role in supporting clients with chronic diseases, such as Type II diabetes or hypertension, by providing non-clinical care coordination, health coaching, and navigation support. This role involves assessing clients' health and social needs, developing individualized plans to achieve health goals, and addressing barriers to care.
Key Responsibilities:
- Provide non-clinical care coordination and health coaching to clients with chronic diseases
- Assess clients' health and social needs and develop individualized plans to achieve health goals
- Address barriers to care and provide health education and coaching to achieve improved client-level health outcomes
Requirements and Qualifications:
- Entry-level knowledge and basic employment skills
- 1-3 years of experience
- Maryland Peer Recovery Specialist Certification (CPRS)
