Job Title: Community Health Navigator

At LifeBridge Health, we are seeking a skilled Community Health Navigator to join our team. As a Community Health Navigator, you will play a pivotal role in providing non-clinical care coordination, health coaching, and navigation support to clients.

Job Summary:

The Community Health Navigator is responsible for assessing clients' health and social needs, developing and implementing individualized plans to support clients in meeting their health goals, and addressing barriers to care. You will work closely with clients to provide health education and coaching to achieve improved client-level health outcomes.

Requirements and Qualifications:
  • Entry-level knowledge and basic employment skills
  • High school diploma or GED equivalent
  • 1-3 years of experience in a related field
  • Maryland Peer Recovery Specialist Certification (CPRS)
