Job Summary:

This position performs assessments, treatment planning, and discharge planning under general supervision. The incumbent provides counseling and therapy to clients requiring complex intervention, coordinates and monitors the assessment and treatment planning process, and provides social support to cope with mental illness and disabilities.

  • Conduct intake, screening, and assessment of clients as appropriate.
  • Provide effective clinical assessments by gathering relevant information to identify problems and determine eligibility criteria.
  • Develop person-centered plans for service users.
  • Establish individual treatment plans outlining goals of treatment based on assessment.
  • Provide individual, family, or group counseling/training to facilitate recovery.
  • Evaluate, facilitate, and monitor services and client progress in relation to established goals and objectives.
  • Maintain confidentiality of information according to established departmental policies and procedures.
  • Adjust techniques and interventions as needed to ensure progress and recovery.
  • Ensure current and authorized treatment plans before providing services.
  • Deliver services that meet best practice guidelines and consumer needs.
  • Oversee discharge planning process.
  • Coordinate with the interdisciplinary team to assist service users, including behavioral health specialists, psychiatrists, nurses, quality assurance specialists, paraprofessionals, and directors.
  • Record services accurately and complete required clinical documentation according to agency standards.
  • Maintain documentation in accordance with applicable policies, laws, and instructions.
  • Ensure services provided are within guidelines and document care in compliance with agency requirements and standards.
  • Ensure notes are in CareLogic and signed no later than 10:00 A.M. the day after the service was provided.
  • Maintain a minimum chart audit score of 93% or better for all consumers on caseload.
  • Ensure weekly reports have been addressed and cleaned up within 48 hours of receiving reports.
  • Meet minimum direct time requirements of 1,200 individual billed hours a year.

Required Education and Experience:

A Master's degree from an accredited program and two years of related experience, or a Master's degree from an accredited program and pursuing licensure in the area of assignment. A valid GA driver's license is required.

Work Environment:

This job operates in a variable business setting with trips into the community. The role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines. The role provides basic employment support, which requires the employee to perform in loud/quiet environments, outdoors/indoors, etc. Some minor travel between Gateway sites and in the community is required.

The employee is frequently required to walk, sit, stand, or kneel and occasionally required to climb or balance and stoop. The employee must frequently lift and/or move up to 15 pounds. Lighting and temperature are adequate, and there are not hazardous or unpleasant conditions caused by noise, dust, etc. The employee must be able to travel between Gateway sites.
