Position Overview:
We are looking for a dedicated and empathetic Nurse Practitioner to join our esteemed team.

In this role, you will be instrumental in delivering comprehensive healthcare to patients residing in long-term care facilities.

This position presents a valuable opportunity to positively impact the lives of patients while collaborating with a committed group of healthcare professionals.


Key Responsibilities:
• Deliver comprehensive healthcare services and manage patient care coordination
• Collaborate closely with a multidisciplinary team of healthcare providers
• Evaluate, diagnose, and treat the physical and mental health of residents
• Conduct detailed patient evaluations, including medical history assessments and physical examinations
• Order and analyze diagnostic tests
• Formulate precise diagnoses and treatment strategies
• Prescribe medications and implement treatment protocols
• Provide guidance and support to residents and their families
• Work with the interdisciplinary team to ensure seamless care coordination
• Oversee chronic conditions and track patient progress
• Adjust care plans as necessary
• Commit to person-centered care principles
• Foster a safe and supportive environment for elderly residents in care facilities

Qualifications:
• Valid Nurse Practitioner License
• DEA License
• Minimum of 6 months experience in long-term care settings

Salary Range:
• $90,000 - $120,000.00
