Job Summary

We are seeking a skilled Social Worker to join our team at Delta-T Group. As a Social Worker, you will play a vital role in supporting youth and families in our program, utilizing your expertise to help sustain program requirements and ensure client success.

Responsibilities
  • Provide leadership and support to program structure and youth within the program
  • Empower and support youth and families through initiative and engagement
  • Ensure accurate and secure client files, records, and Individual Service Plans
  • Participate in live supervision with clients and families
  • Meet regularly with the Program Manager for supervision and guidance
  • Engage and lead in weekly staff meetings
  • Assist clients with discharge and aftercare planning
  • Provide timely and accurate documentation of services provided
  • Oversee and review client assessments and develop individual treatment plans
Requirements
  • Master's Degree in Social Work (MSW) required
  • 1-5 years of experience in the Human Services field, preferably working with adolescents and/or young adults
  • Expert knowledge of issues surrounding homelessness
  • Demonstrated ability to work with community leaders and conceptualize complex projects
