Alternate Solutions Health Network LLC is seeking a skilled Physical Therapist to join our team in Chillicothe, Ohio.

Job Summary:

We are looking for a compassionate and dedicated Physical Therapist to provide high-quality care to our patients in the comfort of their own homes. As a Physical Therapist with our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Responsibilities:
  • Provide skilled physical therapy services to patients in their homes, as ordered by the attending physician.
  • Develop and implement individualized plans of care to meet the unique needs of each patient.
  • Conduct thorough assessments and evaluations to identify patient needs and goals.
  • Collaborate with other healthcare professionals to ensure comprehensive care and coordination.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions.
  • Document all aspects of patient care, including progress notes, treatment plans, and discharge summaries.
Requirements:
  • Current licensure as a Physical Therapist in the state of Ohio.
  • Minimum of one year of experience as a Physical Therapist in an acute setting.
  • Home care experience preferred.
  • Ability to effectively communicate and create positive impressions with patients, families, and healthcare teams.
  • Knowledgeable on therapy best practices and able to make appropriate physical therapy judgments.
  • Ability to identify and address patient needs, and develop solutions to meet those needs.
  • Detail-oriented and observant, with the ability to prioritize and be timely.
  • Valid driver's license and auto insurance with your name as a listed driver.
