POSITION OVERVIEW:

This role entails delivering essential nursing services at a healthcare facility, specifically within the Long-Term Care sector. The individual in this position will provide fundamental nursing care to patients, may take on supervisory duties with limited oversight, and can execute advanced technical nursing procedures.

Work is conducted under the general guidance of a Registered Nurse, Physician, or other senior healthcare professionals. The incumbent may also oversee the activities of Hospital/Institutional Aides as necessary.



KEY RESPONSIBILITIES:


- Maintain and manage medical equipment utilized in patient care.
- Administer and accurately document medication delivery.
- Assist with or perform various treatments and medical procedures.
- Educate and support patients and their families regarding health care as outlined in care plans.
- Measure and record vital signs and patient weights.
- Provide internal feedings as required.
- Monitor and assess patient conditions, documenting observations in line with nursing standards.
- Communicate with the supervising nurse regarding patient care and status updates.
- Report any incidents to the supervising nurse.
- Foster positive relationships with patients, visitors, and healthcare staff.
- Assume charge responsibilities when necessary.
- Ensure patient safety and comfort at all times.
- Perform duties typically assigned to Hospital/Institutional Aides.
- Transport patients within the facility as needed.
- Engage in ongoing professional development through in-service education.


ADDITIONAL DUTIES FOR LONG-TERM CARE FACILITY:

- Maintain communication with the supervising Registered Nurse, reporting any changes in resident conditions.
- Administer and document medication, including IV therapies.
- Conduct EKG tests, pulse oximetry assessments, and phlebotomy procedures.


REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES:
- Proficient understanding of basic and advanced nursing techniques.
- Familiarity with infection control protocols and medication administration.
- Ability to implement prescribed therapeutic treatments.
- Competence in following detailed oral and written instructions.
- Skill in record-keeping and report preparation.
- Strong interpersonal skills and professional judgment.
- Capability to perform essential job functions with or without reasonable accommodation.

MINIMUM REQUIREMENTS:
- Completion of a nursing program from an accredited School of Practical Nursing recognized by the New York State Education Department.
- A valid license and current registration as a Licensed Practical Nurse from the New York State Education Department at the time of appointment, or:

- A limited Graduate Practical Nurse (GPN) permit issued by the New York State Education Department at the time of appointment, with a Licensed Practical Nurse license obtained within one year.


SPECIAL REQUIREMENTS FOR ECMCC POSITIONS:

- Certification in Basic Life Support (BLS) from an approved provider at the time of appointment, maintained throughout employment.


SPECIAL REQUIREMENTS FOR HEALTH DEPARTMENT POSITIONS:
- A valid New York State Driver's license and access to a personal vehicle at the time of appointment, maintained throughout employment.
