Key Responsibilities- Provide patient and family advocacy to ensure their needs are met and their voices are heard.
- Coordinate discharge planning to ensure a smooth transition from hospital to home or other care settings.
- Develop and implement psychosocial interventions to address patients' complex health needs.
- Collaborate with the clinical team and medical provider to ensure comprehensive care.
- Identify and connect patients with community resources to support their health and well-being.
- Master's degree in Social Work (MSW) required.
- 2 years of hospital social work or community social services experience preferred.
- LMSW required; LCSW/LCSW-C preferred.
- 3-5 years of experience in a related field.
- MD Social Work License per level of education.
