Key Responsibilities:- Assess patient and family psychosocial needs to develop an effective discharge plan.
- Collaborate with care providers and third-party payors to ensure timely and efficient access to services and resources.
- Provide brief counseling and crisis intervention techniques to remove barriers for a safe and collaborative aftercare plan.
- Stay abreast of changing care management and social work clinical trends, regulatory matters, and third-party payer requirements.
- Master in Social Work from an accredited Social Work University.
- LMSW or LCSW-C licensed in the state of Maryland.
