About the Role

We are seeking a skilled and compassionate Hospice Community Liaison to join our team at Mercy. As a key member of our community outreach team, you will be responsible for building relationships with healthcare providers, patients, and families to promote our Hospice and Home Health services.

Key Responsibilities
  • Develop and maintain relationships with healthcare providers, discharge planners, and other stakeholders to promote our services and facilitate timely referrals.
  • Provide education and support to patients, families, and caregivers on our Hospice and Home Health services, including benefits, eligibility, and access.
  • Collaborate with our clinical team to ensure seamless transitions of care and high-quality patient outcomes.
  • Participate in community outreach and education initiatives to raise awareness about our services and promote our brand.
  • Conduct regular visits to healthcare facilities, nursing homes, and other community settings to promote our services and build relationships.
Requirements
  • Registered Nurse (RN) license in Missouri
  • Minimum three years of nursing experience, with a focus on Hospice and Home Health
  • Excellent communication, interpersonal, and problem-solving skills
  • Ability to work independently and as part of a team
  • Basic computer skills and familiarity with electronic medical records
Preferred Qualifications
  • Certification in Hospice and Palliative Nursing (CHPN)
  • Experience in providing Hospice and Home Health nursing care
