Qualifications:

  • Experience in a relevant field, such as general practice, family or internal medicine, urology, or endocrinology is preferred.
  • No current or pending malpractice claims.
  • Possession of an active state license in good standing.
  • Active DEA Certificate is preferred but not mandatory.
  • Active Controlled Substance Registration.
